About The Position

Concierge Home Care is hiring a Home Health Physical Therapist (PT). This role offers more control over your schedule, meaningful patient relationships, and the ability to increase income based on productivity. The coverage area is Lakeland / Polk County, FL, with territories designed to minimize drive time through clustered scheduling. The position emphasizes AI-assisted charting to reduce documentation time, a user-friendly EMR (WellSky/Kinnser), and structured onboarding with mentorship. Employment is contingent upon successful completion of an AHCA Level 2 background screening.

Requirements

• Active, unencumbered Florida Physical Therapist license
• Valid driver’s license, reliable transportation, and auto insurance

Responsibilities

• Evaluate patients and develop individualized treatment plans
• Provide therapeutic exercises and mobility training
• Educate patients and caregivers on safety and recovery
• Collaborate with physicians and interdisciplinary teams
• Supervise Physical Therapist Assistants (PTAs)
• Complete documentation using streamlined technology
