About The Position
As a Home Health Physical Therapist, you’ll work one-on-one with patients in their homes—helping them regain independence while you enjoy full autonomy over your workday. No crowded waiting rooms. No shift work. Just impactful, patient-centered care. This role is located in Orange Park, FL, serving North & Northeast Florida.
Requirements
- Active PT license (or eligible for Florida PT licensure / Compact privilege)
- Valid driver’s license, reliable transportation, and auto insurance
- Passion for high-quality, autonomous patient care
Responsibilities
- Work one-on-one with patients in their homes.
- Help patients regain independence.
- Provide autonomous, patient-centered care.
