Concierge Home Care is hiring a Home Health Physical Therapist (PT) in the Palm Beach County area. This role offers more control over your schedule, meaningful patient relationships, and the ability to increase your income based on productivity. The coverage area is Indian River County, with territories designed to minimize drive time through clustered scheduling. The position utilizes a pay-per-visit model with an average full-time earning potential of $95,000 – $120,000+, with top performers earning over $150K annually. There are quarterly bonus opportunities, mileage reimbursement, and a data plan stipend. The company provides consistent patient volume. Therapists choose Concierge Home Care for its flexibility and autonomy, allowing them to create and manage their own schedules with no weekends or holidays required (optional flexibility), no hospital shifts or 12-hour rotations, and greater control over work-life balance. Requirements

• Active, unencumbered Florida Physical Therapist license
• Valid driver’s license, reliable transportation, and auto insurance

Responsibilities

• Evaluate patients and develop individualized treatment plans
• Provide therapeutic exercises and mobility training
• Educate patients and caregivers on safety and recovery
• Collaborate with physicians and interdisciplinary teams
• Supervise Physical Therapist Assistants (PTAs)
• Complete documentation using streamlined technology
