About The Position
Concierge Home Care is seeking a Home Health Physical Therapist Assistant (PTA) to join their team. This position offers flexibility, independence, and the opportunity to provide meaningful one-on-one care in patients' homes. The role involves assisting patients in improving mobility, strength, and independence, collaborating with patients and caregivers, monitoring progress, and completing documentation using AI-assisted tools. The coverage area is Orange Park/Clay County, with territories designed to minimize drive time.
Requirements
- Active, unencumbered Florida Physical Therapist Assistant (PTA) license
- Valid driver’s license, reliable transportation, and auto insurance
Nice To Haves
- Home health experience preferred, but not required
Responsibilities
- Provide hands-on therapeutic care following the plan of care established by the Physical Therapist
- Assist patients in improving mobility, strength, and independence
- Collaborate with patients, caregivers, and interdisciplinary teams
- Monitor patient progress and communicate updates to supervising PT
- Complete documentation using streamlined, AI-assisted tools
