About The Position

Concierge Home Care is hiring a Home Health Physical Therapist (PT). The coverage area includes Marianna, FL and Jackson County, FL, with territories designed to minimize drive time through clustered scheduling. The position emphasizes AI-assisted charting for reduced documentation time and a user-friendly EMR system (WellSky/Kinnser) to allow more time with patients. Requirements

• Active, unencumbered Florida Physical Therapist license
• Valid driver’s license, reliable transportation, and auto insurance
• Successful completion of an AHCA Level 2 background screening

Responsibilities

• Evaluate patients and develop individualized treatment plans
• Provide therapeutic exercises and mobility training
• Educate patients and caregivers on safety and recovery
• Collaborate with physicians and interdisciplinary teams
• Supervise Physical Therapist Assistants (PTAs)
• Complete documentation using streamlined technology
