About The Position

Concierge Home Care is seeking a Home Health Physical Therapist (PT) for a modified full-time role with a flexible schedule, working 4 days a week. This position offers the opportunity for meaningful patient relationships, increased income based on productivity, and greater control over work-life balance. The role involves 1:1 patient care in the home environment within the Marianna, FL / Jackson County, FL coverage area, with territories designed to minimize drive time. The company utilizes AI-assisted charting and a user-friendly EMR (WellSky/Kinnser) to reduce documentation time, allowing therapists to focus more on patient care. Concierge Home Care also provides structured onboarding, mentorship, ongoing training, free CEUs, and career advancement opportunities.

Requirements

• Active, unencumbered Florida Physical Therapist license
• Valid driver’s license, reliable transportation, and auto insurance
• Successful completion of an AHCA Level 2 background screening

Responsibilities

• Evaluate patients and develop individualized treatment plans
• Provide therapeutic exercises and mobility training
• Educate patients and caregivers on safety and recovery
• Collaborate with physicians and interdisciplinary teams
• Supervise Physical Therapist Assistants (PTAs)
• Complete documentation using streamlined technology
