Physical Therapist-Home Care (Per Diem, as needed))
About The Position
The Physical Therapist provides personalized, evidence-based physical therapy to help home-care patients meet their goals, improve function, and enhance quality of life. The role includes assessing patients and developing and implementing individualized treatment plans, as well as supporting patients and caregivers with therapeutic activities, equipment, and follow-up plans after discharge. This per-diem position is with Rochester Regional Health Home Care in Rochester, New York. The schedule is Monday through Friday, with hours listed as 8 a.m. to 4 p.m. The organization describes its home-care agency as a CMS 5-Star Quality of Patient Care provider and a U.S. News & World Report “High Performing Home Care Agency.”

Responsibilities
- Provide quality, evidence-based care to meet patient goals and needs.
- Assess patients and develop and implement individualized treatment plans.
- Administer manual exercises and instruct, encourage, and assist patients with non-manual exercises, ambulatory functional activities, and activities of daily living.
- Design home exercise programs to support continuation of the therapeutic plan after discharge.
- Instruct patients, families, and caregivers in home exercise programs.
- Recommend and provide assistive equipment.
- Recommend outpatient or home-health follow-up programs.
- Support development of physical therapy staff by providing information.
- Develop and conduct in-service training programs.
