About The Position

Metro Physical & Aquatic Therapy is seeking a motivated and passionate per diem Physical Therapist to provide in-home care in Glen Cove and surrounding towns. The role is open to all experience levels, including new graduates, who will receive training and mentoring. The therapist evaluates patients’ needs, develops individualized treatment plans, delivers therapy to restore function and promote independence, and coordinates care with physicians, patients, families, and interdisciplinary team members. The position offers flexible scheduling and geographic autonomy, with 1099 and W-2 work options. Compensation is $70–$90 per visit, depending on relevant experience, knowledge, skills, other job-related qualifications, and geography.

Requirements

Graduate of a Physical Therapist program.
Licensed Physical Therapist in New York State or eligible for licensure.
Ability to travel to patients’ homes within the assigned geographic territory.

Nice To Haves

Home health experience is preferred but not required.

Responsibilities

Accept and treat patients within the designated geographic area.
Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans.
Develop plans of care and treatment goals in consultation with the physician, patient, and family.
Plan, prepare, and implement customized therapy programs to restore function and promote independence.
Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
Provide timely and accurate documentation for each visit.
Communicate effectively with patients, families, and interdisciplinary team members, especially regarding discharge planning.
Deliver care in compliance with agency policies and professional practice standards.
