Home Health Physical Therapist-Per Diem
About The Position
CenterWell Home Health is seeking a per-diem Home Health Physical Therapist to provide personalized physical therapy to patients in their homes. Reporting to the Clinical Manager, the therapist helps patients regain strength, mobility, independence, and physical function. The role includes planning and administering prescribed treatment, assessing patients, developing rehabilitation programs consistent with physician treatment plans and patient goals, and coordinating care with the patient’s healthcare team. The position is patient-facing and requires TB screening. It also requires a valid state driver’s license and personal vehicle liability insurance meeting the applicable minimum coverage requirements.
Requirements
- Current and unrestricted Physical Therapy license.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Valid state driver’s license.
- Maintain personal vehicle liability insurance that meets the applicable state minimum limits or the stated alternative limits, whichever is higher.
- Complete TB screening if selected for the role.

Responsibilities
- Plan and administer prescribed physical therapy treatment for patients in their homes to help them attain the highest possible level of physical function.
- Assess and document patients’ physical strengths to support physician evaluation and guide treatment goals.
- Develop and implement rehabilitation programs consistent with the physician’s Plan of Treatment and the patient team’s goals.
- Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
- Promptly and thoroughly document patient observations, interventions, and evaluations.
- Report patient progress to the physician, Clinical Manager, care team, patient, and family.
- Submit required evaluations, plans, progress notes, and discharge summaries.
