About The Position

CenterWell Home Health is seeking a per-diem Home Health Physical Therapist to help patients regain strength, mobility, and independence in their homes. Reporting to the Clinical Manager, the therapist provides personalized care focused on functional improvement and works with patients, families, physicians, and the broader care team. The role includes administering physician-prescribed treatment, developing rehabilitation plans, guiding support staff, and completing clinical documentation. The position is patient-facing and requires TB screening, a valid state driver's license, and personal vehicle liability insurance that meets the stated requirements.

Requirements

Hold a current and unrestricted Physical Therapy license.
Maintain current CPR certification.
Have experience collaborating with a team of healthcare professionals.
Have a valid state driver's license and maintain personal vehicle liability insurance meeting applicable requirements.
Complete required tuberculosis screening if selected for the role.

Responsibilities

Plan and administer prescribed physical therapy treatment to patients in their homes to help them attain the highest possible level of physical function.
Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
Develop and implement rehabilitation programs consistent with the physician's Plan of Treatment and the patient team's goals.
Provide PTAs and Home Health Aides with written care instructions and oversee their performance and documentation.
Promptly and thoroughly document patient observations, interventions, and evaluations.
Report patient progress to the physician, Clinical Manager, care team, patient, and family.
Submit required evaluations, plans, progress notes, and discharge summaries.
