About The Position

Adoration Home Health is seeking a dedicated Physical Therapist to provide patient-centered home health care in the Anmoore, West Virginia coverage area. The position offers one-to-one patient care, flexible scheduling options, independent work with team support, and a $20,000 sign-on bonus for external candidates only.

The Physical Therapist evaluates and treats patients according to physician orders, develops individualized treatment plans and measurable goals, and helps patients reach their maximum independence and function. The role includes coordinating with patients, families, physicians, and the treatment team; documenting services in a timely and thorough manner; and supervising Physical Therapist Assistants and Physical Therapy Aides in accordance with the state practice act.

Requirements

• Graduate of a Physical Therapist education program approved by CAPTE.
• Current Physical Therapist licensure in the state.
• American Heart Association CPR certification; certification may be offered and completed upon hire.
• Two years of experience in adult physical therapy.
• Excellent observation, verbal and written communication, problem-solving, and basic math skills.
• Physical therapy skills as assessed by the competency checklist.

Nice To Haves

• Home health experience preferred.

Responsibilities

• Perform physical therapy evaluations and provide treatment based on physician orders, including recommending and procuring adaptive equipment.
• Establish measurable goals and develop treatment plans to help patients achieve their maximum level of independence and function.
• Maintain communication among the treatment team, physician, patient, and family members, as applicable.
• Complete thorough and timely documentation of services to support proper billing.
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with the state practice act.
