About The Position

Adoration Health is seeking a dedicated Home Health Physical Therapist to provide expert, patient-centered care in Moulton, Alabama, and the surrounding coverage area. The role offers one-to-one patient care, flexible scheduling options, independent work with team support, and opportunities for advancement. The position is full-time and offers a $10,000 sign-on bonus to external candidates only. Benefits may vary by employment status.

Requirements

• Bachelor’s degree in Physical Therapy from a college or university with an accredited Physical Therapy program.
• At least two years of appropriate experience as a physical therapist; new graduates may be considered in select markets based on program availability.
• Valid license in the state of practice.
• Current CPR certification, maintained throughout employment.

Nice To Haves

• Community or hospice experience is preferred.

Responsibilities

• Evaluate and treat patients based on physician orders, including recommending and procuring adaptive equipment.
• Establish measurable goals and develop treatment plans to help patients reach their maximum level of independence and function.
• Maintain communication with the treatment team, physician, patient, and family members, as applicable.
• Complete thorough and timely documentation of services to support proper billing.
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities, in accordance with the state practice act.
