About The Position

Adoration Home Health is seeking a dedicated Home Health Physical Therapist to provide patient-centered care in Moulton, Alabama. The role provides one-to-one care in patients’ homes, with flexible scheduling options, independent work, and support from a team. The position is full-time and offers a $10,000 sign-on bonus to external candidates only.

The Physical Therapist evaluates and treats patients in accordance with physician orders, develops measurable goals and treatment plans, and supports patients in reaching their maximum independence and function. The therapist communicates with the treatment team, physicians, patients, and family members; documents services for billing; and supervises Physical Therapist Assistants and Physical Therapy Aides in accordance with the state practice act.

Requirements

• Bachelor’s degree in Physical Therapy from a college or university with an accredited Physical Therapy program.
• At least two years of appropriate experience as a physical therapist; new graduates may be considered in select markets based on program availability.
• Valid license in the state of practice.
• Current CPR certification.

• Community or hospice experience is preferred.

Responsibilities

• Evaluate and treat patients based on physician orders, including recommending and procuring adaptive equipment.
• Establish measurable goals and develop treatment plans to help patients reach their maximum level of independence and function.
• Maintain communication among the treatment team, physician, patient, and family members, as applicable.
• Maintain thorough and timely documentation of services provided to support proper billing.
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with the state practice act.
