About The Position

Adoration Home Health is seeking a dedicated Physical Therapist to provide patient-centered home health care in the Bayonne, New Jersey coverage area. The role provides one-to-one care in patients’ homes, with flexible scheduling options, independent work, and team support. The position is full-time and offers a $10,000 sign-on bonus to external candidates only. The listed pay is $55.00 per hour.

The Physical Therapist evaluates and treats patients according to physician orders, develops measurable treatment goals, and helps patients work toward their greatest possible independence and function. The role also includes coordinating with the care team, physicians, patients, and families; maintaining timely documentation; and supervising Physical Therapist Assistants and Physical Therapy Aides in accordance with the state practice act.

Requirements

• Graduate of a Physical Therapist education program approved by CAPTE.
• Current Physical Therapist licensure in the state of practice.
• American Heart Association CPR certification; certification may be offered and completed upon hire.
• Two years of experience in adult physical therapy.
• Excellent observation, verbal and written communication, problem-solving, and basic math skills.
• Physical therapy skills as described in the competency checklist.

• Home health experience preferred.

Responsibilities

• Evaluate and treat patients based on physician orders, including recommending and procuring adaptive equipment.
• Establish measurable goals and develop treatment plans to help patients achieve their maximum level of independence and function.
• Maintain communication among the treatment team, physician, patient, and family members as applicable.
• Document services thoroughly and promptly to support proper billing.
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with the state practice act.
