About The Position

Prime Home Health is hiring full-time and PRN Physical Therapists to serve San Antonio and surrounding areas. The role provides individualized rehabilitation services to patients in their homes, with the goal of improving mobility, strength, balance, safety, independence, and quality of life. Therapists work independently in the home setting while coordinating care with patients, families, physicians, and the interdisciplinary home health team.

Requirements

• Current Texas Physical Therapist license or eligibility to practice in Texas.
• Bachelor's, Master's, or DPT degree from an accredited Physical Therapy program.
• Current CPR/BLS certification.
• Strong clinical, organizational, communication, and interpersonal skills.
• Ability to work independently, manage a daily schedule, and provide excellent patient care.
• Reliable transportation and ability to travel throughout an assigned service area.

Nice To Haves

• Minimum 2 years of home health experience preferred.
• Bilingual English/Spanish ability is a plus.
• Experience with WellSky/Kinnser, OASIS, and home health documentation preferred.

Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans.
• Provide therapeutic exercises, gait and balance training, strengthening, mobility training, and other appropriate interventions.
• Help patients improve functional independence and safely perform daily activities.
• Educate patients and families on exercises, safety, fall prevention, and home programs.
• Monitor patient progress and communicate changes with physicians and the interdisciplinary care team.
• Maintain timely and accurate clinical documentation and electronic medical records.
• Coordinate care with nurses, physicians, OTs, SLPs, and other members of the home health team.
• Provide compassionate, patient-centered care while working independently in the home setting.
