About The Position

Pinnacle Home Care is hiring a full-time Physical Therapist Assistant (PTA) to provide compassionate, patient-centered care in patients’ homes. The role supports physical therapists by helping evaluate patients, carry out care plans, monitor progress, and coordinate care with patients, families, physicians, and other healthcare staff. The position is with a home health provider serving patients across Florida.

Requirements

• Graduate of an accredited Physical Therapy Assistant program.
• Licensed Physical Therapist Assistant in Florida.
• Current BLS certification.
• Reliable transportation, a valid driver’s license, and vehicle insurance.
• Strong physical stamina and excellent communication skills.

Nice To Haves

• One year of home healthcare experience is preferred; new graduates are welcome to apply.
• Experience with electronic medical records is a plus.

Responsibilities

• Assist in evaluating patients, developing care plans, and conducting periodic reassessments.
• Document clinical and daily progress notes for review and submission by the physical therapist.
• Perform therapeutic exercises and assist patients with self-help devices.
• Instruct patients and families on therapy exercises alongside the physical therapist.
• Monitor and report adverse drug reactions or ineffective therapies to the physical therapist.
• Observe, document, and report clinical findings to the physical therapist and physician as needed.
• Coordinate with physical therapy and nursing staff on patient discharge plans and summaries.
• Educate health team members and families to support patient care.
• Maintain accurate documentation and comply with agency policies.
