About The Position

Pinnacle Home Care is hiring a full-time Physical Therapist Assistant (PTA) to provide compassionate, patient-centered care in patients’ homes. The role supports physical therapists in evaluating patients, developing and carrying out care plans, monitoring progress, and coordinating care with patients, families, physicians, and other health team members. Pinnacle Home Care is a Medicare-certified home health provider serving patients across Florida.

Requirements

Graduate of an accredited Physical Therapy Assistant program.
Licensed Physical Therapist Assistant in Florida.
Current BLS certification.
Reliable transportation, a valid driver’s license, and vehicle insurance.
Strong physical stamina and excellent communication skills.

Nice To Haves

One year of home healthcare experience is preferred; new graduates are welcome to apply.
Experience with electronic medical records.

Responsibilities

Assist with patient evaluations, development of care plans, and periodic reassessments.
Document clinical and daily progress notes for review and submission by the physical therapist.
Perform therapeutic exercises and assist patients with self-help devices.
Instruct patients and families on therapy exercises alongside the physical therapist.
Monitor and report adverse drug reactions or ineffective therapies to the physical therapist.
Observe, document, and report clinical findings to the physical therapist and physician as needed.
Coordinate with the physical therapist and nursing staff on patient discharge plans and summaries.
Educate health team members and families to support patient care.
Maintain accurate documentation and comply with agency policies.
