About The Position

Pinnacle Home Care is hiring a full-time Physical Therapist Assistant (PTA) to provide compassionate, patient-centered home health care in Florida. The PTA helps patients heal and regain independence by delivering individualized care in their homes. The role works with physical therapists, nursing staff, patients, families, and other health team members to support treatment and care planning. Pinnacle Home Care is Florida’s largest Medicare-certified home health provider and has delivered high-quality care for over two decades.

Requirements

• Graduate of an accredited Physical Therapy Assistant program.
• Current Florida PTA license.
• Current BLS certification.
• Reliable transportation, a valid driver’s license, and insurance.
• Strong physical stamina and excellent communication skills.

Nice To Haves

• One year of home healthcare experience is preferred; new graduates are welcome to apply.
• Experience with electronic medical records is a plus.

Responsibilities

• Assist with evaluating patients, developing care plans, and conducting periodic reassessments.
• Document clinical and daily progress notes for physical therapist review and submission.
• Perform therapeutic exercises and assist patients with self-help devices.
• Instruct patients and families on therapy exercises alongside the physical therapist.
• Monitor and report adverse drug reactions or ineffective therapies to the physical therapist.
• Observe, document, and report clinical findings to the physical therapist and physician as needed.
• Coordinate with physical therapy and nursing staff on patient discharge plans and summaries.
• Educate health team members and families to support patient care.
• Maintain accurate documentation and comply with agency policies.
