About The Position

As a therapist at CenterWell Home Health, reporting to the Clinical Manager, you will help patients regain strength, mobility, and independence in their homes. You will provide personalized care focused on functional improvement and help patients overcome limitations and improve their quality of life. The role involves delivering prescribed physical therapy in patients’ homes, coordinating with physicians and the patient care team, and documenting treatment and progress. This is a patient-facing position subject to Humana’s TB screening program. The role is also covered by Humana’s driver safety program and requires a valid state driver’s license and personal vehicle liability insurance that meets applicable minimum limits.

Requirements

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid state driver’s license.
• Maintain personal vehicle liability insurance in accordance with the state’s required minimum limits or the stated coverage limits, whichever is higher.
• Required to undergo TB screening if selected for the role.

Responsibilities

• Plan and administer prescribed physical therapy treatment for patients in their homes to help them attain the highest possible level of physical function.
• Assess and document patients’ physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement rehabilitation programs consistent with the physician’s Plan of Treatment and patient-team goals.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
• Promptly and thoroughly document patient observations, interventions, and evaluations.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family.
• Submit required evaluations, plans, progress notes, and discharge summaries.
