Physical Therapist - Home Health - Full-Time
About The Position
Therapy Partner Solutions (TPS), a therapist-owned and operated organization, provides home health therapy services for Accent Care Home Health. The Physical Therapist will work with a diverse caseload as part of a collaborative, multidisciplinary team that includes physical and occupational therapists, physicians, and other healthcare professionals to deliver coordinated, patient-centered care in patients’ homes in Jacksonville, Florida. The role offers defined territories and thoughtful scheduling to support appropriate caseloads, limited drive time between visits whenever possible, and clinical leadership from therapists. Individualized support and professional development opportunities are available, including mentorship, advanced training and certifications, educational programming, and an OCS preparation collaborative. The position is listed as full-time, with a per-visit rate of $60–$120 per hour. Full-time benefit options begin at 30 hours per week.
Requirements
- Graduate, or be currently or pending graduation, from an accredited physical therapy program.
- Have an active or pending state physical therapist license.
- Be comfortable working independently in the home setting.
