About The Position
Therapy Partner Solutions (TPS) is hiring a full-time Physical Therapist to provide home health therapy services in Jacksonville, Florida, for Accent Care Home Health. TPS is therapist-owned and operated and works with a multidisciplinary team of physical and occupational therapists, physicians, and other healthcare professionals to deliver coordinated, patient-centered care. The role involves working with a diverse caseload in patients’ homes, with support from clinical leadership and collaborating colleagues. The position offers thoughtful scheduling, defined territories, and efforts to limit drive time between visits. Clinicians receive individualized support for professional interests and growth, along with opportunities for mentorship, advanced training, certifications, virtual education, and OCS preparation. The listed per-visit rate is $60–$120 per hour.
Requirements
- Graduate, or be a current or pending graduate, of an accredited physical therapy program.
- Hold an active or pending state physical therapist license.
- Be comfortable working independently in the home setting.
Responsibilities
- Provide physical therapy services to home health patients.
- Work with a diverse caseload in the home setting.
- Collaborate with physical and occupational therapists, physicians, and other healthcare professionals to support coordinated, patient-centered care.
- Work independently while collaborating with AccentCare Home Health colleagues and clinical leadership.
