About The Position

Integrity Home Care + Hospice is recruiting a compassionate, full-time Home Health Physical Therapist to serve patients in Springfield, Missouri, and surrounding areas. The therapist provides one-to-one clinical care in clients’ homes, helping them recover from injury, illness, surgery, or hospitalization. The position includes a $30,000 retention bonus for full-time employees. Training is provided for candidates who have not previously worked in home health.

Requirements

Active and current Physical Therapist license.
Ability to travel locally to assigned cases.

Nice To Haves

One year of clinical experience is preferred.
Previous home health care experience is welcomed but not required; training is provided.

Responsibilities

Make home visits to clients in designated geographic territories.
Perform diagnostic tests and measurements, including assessments of joint mobility and range, transfer status, stability, gait patterns and appearance, muscle strength and endurance, balance, and safety.
Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence-based dressings and topical agents.
Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance activities, and other interventions.
Assess and revise Physical Therapy care plans, and participate with nursing in multidisciplinary care planning as appropriate.
Educate and instruct clients, family members, or other representatives in rehabilitative care and activities that support health, safety, and independent living.
Document observations, interventions, evaluations, client care management, and services provided using a touch-pad tablet.
