About The Position

InHome Therapy is hiring full-time and part-time licensed Physical Therapists to provide home care to adult and geriatric patients in and around Waukegan. The role offers a flexible schedule, autonomy, a consistent caseload, and clinical and administrative support. This position is paid on a per-visit basis, with an evaluation visit rate ranging from $65 to $85. Final compensation depends on experience, qualifications, and internal equity. The IHT Milestone Reward Program offers up to $15,000 in career milestone payments, paid in $5,000 increments; details are provided at the offer stage. The IHT PT Clinical Excellence Bonus provides additional weekly pay based on visit complexity and volume. Benefits eligibility varies by employment status and weekly hours worked.

Requirements

Active physical therapy (PT) license in good standing.
BLS CPR certification.
Reliable transportation to visit patients in their homes.
Basic comfort with email, texting, and EMR systems; training is provided.

Responsibilities

Deliver in-home physical therapy services to adult and geriatric patients in the community.
Evaluate patient mobility, strength, and function to develop individualized treatment goals.
Implement rehabilitation and therapeutic exercise programs.
Educate patients and families on recovery plans and preventive care.
Maintain accurate and timely home health care documentation in accordance with Medicare and agency standards.
