About The Position

Metro Physical & Aquatic Therapy is seeking a motivated, passionate per diem Physical Therapist to provide in-home care in Lindenhurst and neighboring towns in Western Suffolk County. The role offers flexible scheduling and autonomy, with the ability to see patients any seven days of the week. All experience levels are encouraged to apply, and the company will train the right new graduate. A mentor is available to support documentation and compliance. The position may be offered with 1099 or W-2 work arrangements. The anticipated pay range is $70–$90 per visit, depending on factors including relevant experience, knowledge, skills, other job-related qualifications, and geography.

Requirements

Graduate of a Physical Therapist program.
Licensed Physical Therapist in New York or eligible for licensure.
Ability to travel to patients’ homes within the assigned geographic territory.

Nice To Haves

Home health experience is preferred but not required.

Responsibilities

Accept and treat patients within the designated geographic area.
Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans.
Develop a plan of care and treatment goals in consultation with the physician, patient, and family.
Plan, prepare, and implement customized therapy programs to restore function and promote independence.
Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
Provide timely and accurate documentation for each visit.
Communicate effectively with patients, families, and interdisciplinary team members, especially regarding discharge planning.
Deliver care in compliance with agency policies and professional practice standards.
