Home Care Physical Therapist- Full Time (Western Suffolk)
About The Position
Metro Physical & Aquatic Therapy is seeking a motivated and passionate full-time Physical Therapist to provide in-home care throughout a designated area within Suffolk County. This role is ideal for a Physical Therapist ready to advance their career within a growing organization that offers a progressive, supportive, and innovative team environment. All experience levels are welcome, and training will be provided for the right new graduate. The position offers flexibility and autonomy, allowing therapists to see patients any 7 days of the week, with a preference for focusing on patients within their primary geographic area.
Requirements
- Physical Therapist Program Graduate
- Licensed Physical Therapist in the state of New York or eligible.
- Ability to travel to patients’ homes within the assigned geographic territory
Nice To Haves
- Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
Responsibilities
- Accept and treat patients within your designated geographic area
- Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans
- Develop a plan of care and treatment goals in consultation with the physician, patient, and family
- Plan, prepare, and implement customized therapy programs to restore function and promote independence
- Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
- Provide timely and accurate documentation for each visit
- Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
- Deliver care in compliance with agency policies and professional practice standards
