About The Position

Metro Physical & Aquatic Therapy is seeking a motivated, passionate full-time Physical Therapist to provide in-home care in a designated area of Suffolk County, including Lindenhurst and Dix Hills. The therapist evaluates patients’ physical therapy needs in the home, develops individualized treatment plans, and delivers customized therapy programs to restore function and promote independence. The role involves coordinating care and treatment goals with physicians, patients, and families; educating caregivers; documenting each visit; and collaborating with interdisciplinary team members, particularly on discharge planning. All experience levels are encouraged to apply, and the company will train the right new graduate. A mentor is available to support documentation and compliance.

Requirements

• Graduate of a Physical Therapist program.
• Licensed Physical Therapist in New York State or eligible for licensure.
• Ability to travel to patients’ homes within the assigned geographic territory.

Nice To Haves

• Home health experience is preferred but not required.

Responsibilities

• Accept and treat patients within the designated geographic area.
• Evaluate patients’ physical therapy needs in the home setting and develop individualized treatment plans.
• Develop plans of care and treatment goals in consultation with the physician, patient, and family.
• Plan, prepare, and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, especially regarding discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.
