About The Position

Metro Physical & Aquatic Therapy is seeking a motivated and passionate per diem Physical Therapist to provide in-home care in Dix Hills and neighboring towns in Western Suffolk County. The role is open to candidates at all experience levels, including new graduates, who will receive training and mentorship. The position offers a flexible schedule and autonomy to see patients on any seven days of the week, with preference given to assignments in the therapist’s primary geographic area. Therapists may choose between 1099 and W-2 work arrangements. The listed anticipated compensation is $70–$90 per visit; the posting also contains a separate compensation statement reading “from USD 7090 - hourly.”

Requirements

• Graduate of a Physical Therapist program.
• Licensed Physical Therapist in New York State or eligible for licensure.
• Ability to travel to patients’ homes within the assigned geographic territory.

Nice To Haves

• Home health experience is preferred but not required.

Responsibilities

• Accept and treat patients within the designated geographic area.
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans.
• Develop plans of care and treatment goals in consultation with the physician, patient, and family.
• Plan, prepare, and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, especially regarding discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.
