About The Position

Metro Physical & Aquatic Therapy is seeking a motivated and passionate per diem Physical Therapist to provide in-home care. This is an opportunity for a PT ready to take the next step forward in their career to join a growing organization in a progressive, supportive, and innovative team. All levels of experience are encouraged to apply, and the company will train the right new graduate. The role involves providing 1:1 treatment to patients within a designated geographic area, developing individualized treatment plans, and educating caregivers. The company emphasizes a focus on patient care with quick EMR and offers flexibility and autonomy.

Requirements

• Physical Therapist Program Graduate
• Licensed Physical Therapist in the state of New Jersey or eligible.
• Ability to travel to patients’ homes within the assigned geographic territory

Nice To Haves

• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• First Aid, AED, & CPR Certifications.

Responsibilities

• Accept and treat patients within your designated geographic area
• Provide 1:1 treatment
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care
• Educate family members, home health aides, and other caregivers on proper therapy techniques
• Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit
• Must be willing to treat geriatric patients.
