About The Position

CVHCare is seeking a qualified Physical Therapist to cover assigned county territories, primarily Area #8 (Richmond, Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, Kensington), and potentially Area #2 (Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont) or Area 7 (Vallejo, Benicia, Napa, American Canyon, Yountville). Additional travel outside the assigned area may be required, and weekend coverage might be necessary. The role involves managing patient caseloads with specific productivity requirements or a per-visit payment structure. The Physical Therapist must be willing to see Covid+ and Covid Pui patients. All travel time, visit time, documentation, and communication time with patients and doctors are covered by the rate.

Requirements

• Current CA PT License
• BLS certification
• Valid Driver's License
• Auto insurance
• Auto registration
• Annual TB test
• MMR vaccination
• Varicella vaccination
• HepB vaccination
• TDap vaccination (within 10 years)
• Annual Flu vaccination
• Ability to pass a drug screen
• Physical examination results
• Willingness to travel outside the area or work weekends if required to maintain weekly visit expectations.
• Willingness to see Covid+ and Covid Pui patients.

Responsibilities

• Provide physical therapy services within assigned county territories.
• Travel within the assigned territory and potentially outside of it to meet caseload demands.
• Provide weekend coverage as required.
• Manage patient caseloads according to either a guaranteed productivity model (27 points/week, 36 hours guarantee) or a per-visit payment basis.
• See Covid+ and Covid Pui patients.
• Complete documentation, including OASIS assessments, for patient visits.
• Communicate with patients and doctors as needed.
