About The Position

CVHCare is seeking a qualified Physical Therapist for a position covering specific territories in Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont, Richmond, Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, and Kensington. Additional travel outside these areas may be required, and weekend coverage might be necessary. The role involves seeing both Covid+ and Covid Pui patients, with payment made on a per-visit basis. The agency's contract with the traveler must mirror the confirmation page, and all requested time off must be disclosed at the time of submission. Time off requested after the acceptance date will not be approved.

Requirements

• Current CA PT License
• BLS certification
• Valid Driver's License
• Auto Insurance
• Auto Registration
• Annual TB test
• MMR vaccination
• Varicella vaccination
• HepB vaccination
• TDap vaccination (within 10 years)
• Annual Flu vaccination
• Ability to pass a drug screen
• Pass a physical examination
• Willingness to travel outside the service area occasionally to secure a full caseload (if non-guarantee).
• Willingness to see Covid+ and Covid Pui patients.

Responsibilities

• Provide physical therapy services to patients in assigned territories.
• Travel to patients' homes within specified areas (Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont, Richmond, Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, Kensington).
• Be willing to travel outside the assigned area if required to maintain weekly visit expectations.
• Provide weekend coverage if required.
• See Covid+ and Covid Pui patients.
• Complete documentation, including OASIS assessments (SoC, ReCert, RoC, F/up, Discharge).
• Manage drive/travel time, visit time, documentation time, and time spent on phone calls to patients and doctors.
