About The Position

CVHCare is looking for a qualified Physical Therapist to cover assigned territories in Contra Costa County, California. The role involves providing physical therapy services to patients, potentially including those who are COVID-19 positive or PUI. Payment is on a per-visit basis, with different visit types converting to points and then to approved hours. The rate is all-inclusive, covering travel, visit time, documentation, and communication with patients and doctors. Weekend coverage may be required. Travelers must be willing to travel outside their assigned area if necessary to maintain weekly visit expectations. Specific productivity and guarantee policies apply depending on the contract type (guaranteed vs. non-guaranteed).

Requirements

• Current CA PT License
• BLS certification
• Valid Driver's License
• Auto insurance
• Auto registration
• Annual TB test
• MMR vaccination
• Varicella vaccination
• HepB vaccination
• TDap vaccination (within 10 years)
• Annual Flu vaccination
• Ability to pass a drug screen
• Pass a physical examination

Responsibilities

• Provide physical therapy services to patients within assigned territories (Area #4 and Area #5 in Contra Costa County).
• Travel within the assigned territory and potentially outside of it to meet patient needs.
• Cover weekend shifts if required.
• See COVID+ and COVID PUI patients.
• Complete patient visits, documentation, and phone calls.
• Maintain productivity standards (e.g., at least 27 points per week for guaranteed positions).
• Adhere to specific policies regarding patient refusal, holidays, and time off for guaranteed positions.
• Ensure agency contract mirrors the confirmation page and all requested time off (RTO) is disclosed at the time of submission.
