About The Position

CVHCare is seeking a qualified Physical Therapist for a position covering specific territories in the Fremont and Oakland areas. The role involves providing physical therapy services, potentially including travel outside the assigned areas and weekend coverage. The position offers both guaranteed and non-guaranteed hour options, with payment structured on a per-visit basis that converts to points and then to hours for approval. The rate is all-inclusive, covering travel, visit time, documentation, and communication. The therapist must be willing to see Covid+ and Covid Pui patients.

Requirements

• Current CA PT License
• BLS certification
• Driver's License
• Auto insurance
• Auto registration
• Annual TB test
• MMR vaccination
• Varicella vaccination
• HepB vaccination
• TDap vaccination (within 10 years)
• Annual Flu vaccination
• Ability to pass a drug screen
• Physical examination clearance
• Willingness to travel outside the service area if required.
• Willingness to work weekends if required.
• Willingness to see Covid+ and Covid Pui patients.

Responsibilities

• Provide physical therapy services within assigned territories (Area #1: Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City; Area #2: Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont).
• Travel outside the assigned area may be required to secure a full caseload.
• Weekend coverage may be required.
• Maintain weekly visit expectations.
• See Covid+ and Covid Pui patients.
• Complete documentation, including OASIS assessments (ReCertification, RoC, SoC, Discharge, F/up).
• Manage drive/travel time, visit time, documentation time, and time spent on phone calls to patients and doctors.
