Requirements

Graduate of an American Physical Therapy Association (APTA)-approved physical therapy program.
Current, unrestricted PT license valid for the state of work.
Able and willing to travel within the branch or office coverage area and travel 50%.
Dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.

Responsibilities

Promote Elara Caring’s philosophy, mission statement, and administrative policies to ensure quality of care.
Provide direct patient care, evaluate functional needs and outcomes, consult with other specialists, and adjust care plans to support optimal patient outcomes.
Design and implement patient care plans based on thorough assessments, incorporating participation by significant others when appropriate.
Conduct therapy within defined standards of care.
Direct support staff in helping patients achieve functional goals.
Document evaluations, treatment goals, and plans; update documentation regularly and maintain prescriptions and signatures.
Provide direction and leadership to staff and serve as a resource to promote physical therapy in the community.
