Physical Therapist - Telehealth/Hybrid
About The Position
Innovate Rehab and Wellness is seeking a full-time Physical Therapist to provide patient-centered care in a skilled nursing facility. The therapist will evaluate patients’ movement and functional limitations, create and lead individualized treatment plans, and help patients restore movement, function, and quality of life. The role works closely with an interdisciplinary team to support optimal recovery. The position is based at Onyx Care of Arcadia in Arcadia, Louisiana. The schedule is Monday through Friday and is described as flexible. Pay is a competitive hourly rate based on experience; the salary range is undisclosed. Requirements
- Doctorate or master’s degree in Physical Therapy from an accredited program.
- Valid state physical therapist license.
- Strong communication, problem-solving, and patient-care skills.
Nice To Haves
- Clinical experience is preferred; new graduates are welcome.
- CPR certification is preferred.
Responsibilities
- Perform comprehensive physical therapy evaluations.
- Develop personalized, evidence-based treatment plans.
- Provide hands-on therapy using manual techniques, therapeutic exercise, and mobility training.
- Educate patients and caregivers about home programs and injury prevention.
- Track and document patient progress and adjust treatment plans as needed.
- Collaborate with physicians, nurses, occupational therapists, and other clinicians.
- Stay current with physical therapy best practices through continuing education.
- Ensure compliance with regulatory and ethical standards.
