Physical Therapist - Telehealth/Hybrid
About The Position
Innovate Rehab and Wellness is hiring a full-time Physical Therapist to provide patient-centered care in a skilled nursing facility. The role is based in Arcadia, Louisiana, and is described as telehealth/hybrid. The therapist will evaluate patients, identify movement and functional limitations, develop and lead individualized treatment plans, and help restore movement, function, and quality of life. The position works closely with an interdisciplinary team to support optimal patient recovery. Its care model emphasizes one-on-one treatment, with group and concurrent treatments accounting for under 1%, realistic scheduling, clinical outcomes, patient satisfaction, and professional growth. The schedule is Monday through Friday and flexible. Pay is a competitive hourly rate based on experience.
Requirements
- Doctorate or Master’s degree in Physical Therapy from an accredited program.
- Valid state physical therapist license.
- Strong communication, problem-solving, and patient-care skills.
Nice To Haves
- Clinical experience is preferred, but new graduates are welcome.
- CPR certification is preferred.
Responsibilities
- Perform comprehensive physical therapy evaluations.
- Identify patients’ movement and functional limitations.
- Develop and provide personalized, evidence-based treatment plans.
- Provide hands-on therapy, including manual techniques, therapeutic exercise, and mobility training.
- Educate patients and caregivers about home programs and injury prevention.
- Track and document patient progress and adjust treatment plans as needed.
- Collaborate with physicians, nurses, occupational therapists, and other clinicians.
- Stay current with physical therapy best practices through continuing education.
- Ensure compliance with regulatory and ethical standards.
