Physical Therapist (SIGN ON BONUS) - Cranford, NJ
About The Position
FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in private residences and senior living communities in Cranford and surrounding communities. Through its Geriatric House Calls™ model, clinicians help patients improve mobility, independence, wellness, and quality of life. Care settings include independent living, assisted living, and memory care communities. Clinicians may also provide telehealth or other virtual care when appropriate. The role offers autonomy to create, manage, and adjust a treatment schedule, build lasting patient relationships, and work collaboratively with an interdisciplinary team. FOX describes its practice as clinician-operated and focused on meaningful relationships, evidence-based care, and abolishing ageism. The posting lists full-time, part-time, and PRN/Fox Flex employment pathways. New graduates may be eligible for FOX’s Emerging Professionals Mentor Program, and H1B sponsorship is available for qualified clinicians. The standard New Jersey base pay range for a full-time role is $70,000–$102,000 annually, with the opportunity to earn more. Part-time and FOX Flex compensation is prorated. Requirements
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
- Ability to drive to patient homes and senior living communities.
- Wear PPE when required.

Responsibilities
- Provide one-on-one physical therapy to older adults in private residences and senior living settings.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Deliver therapy sessions that support wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Create, manage, and adjust a treatment schedule.
- Travel to patient homes and senior living communities as part of the Geriatric House Calls™ model.
