Physical Therapist (Sign on Bonus!) - Lincroft, NJ
About The Position
FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in Lincroft, New Jersey, and surrounding communities. Care is delivered in private residences and senior living settings, including independent living, assisted living, and memory care, through the Geriatric House Calls™ model. Clinicians help patients improve mobility, independence, wellness, longevity, and quality of life. The role offers autonomy to create and manage a treatment schedule and build lasting patient relationships. Clinicians may also incorporate telehealth and other virtual care into their schedules. FOX is a clinician-operated practice focused on meaningful relationships, evidence-based care, and abolishing ageism, with an interdisciplinary and clinician-driven culture. Full-time, part-time, and PRN/FOX Flex pathways are available. Full-time roles are salaried with benefits; part-time roles are salaried with benefits eligibility based on minimum hours; PRN/FOX Flex roles are paid per unit. In New Jersey, the standard base pay range for full-time roles is $70,000–$102,000 annually, with the opportunity to earn more; part-time and FOX Flex compensation is prorated. H1B sponsorship is available for qualified clinicians, and new graduates are eligible for FOX’s Emerging Professionals Mentor Program.
Requirements
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with transfers and bed mobility, repositioning patients, extended standing and walking, climbing stairs, and performing therapeutic activities.
- Ability to drive to patient homes and senior living communities.
- Wear PPE when required.

Responsibilities
- Provide one-on-one physical therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Provide therapy that supports patient wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
- Potentially supervise PTAs/OTAs, mentor emerging professionals, teach as a Clinical Instructor, and contribute blog articles or social content for additional compensation.
