About The Position
FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in private residences and Senior Living Communities in Hamilton, New Jersey, and surrounding communities. Using FOX’s Geriatric House Calls™ model, clinicians help patients improve mobility, independence, wellness, and quality of life. The role offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and, when appropriate, incorporate telehealth and other virtual care. FOX offers full-time, part-time, and PRN/Fox Flex employment pathways. New graduates are eligible for the Emerging Professionals Mentor Program, and H1B sponsorship is available for qualified clinicians. FOX Rehabilitation is a clinician-operated practice focused on meaningful relationships, evidence-based care, and its mission to abolish ageism. Clinicians work in a collaborative, interdisciplinary environment and may pursue additional income through referral incentives, clinical instruction, mentoring, supervising assistants, care above quota, and content creation. Full-time base pay in New Jersey is $70,000–$102,000 annually, with the opportunity to earn more; part-time and PRN compensation is prorated. Benefits are available for eligible roles.
Requirements
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
- Ability to drive to patient homes and senior living communities.
- Ability to wear PPE when required.

Responsibilities
- Provide one-on-one physical therapy to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and provide evidence-based treatment.
- Deliver therapy that supports patient wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Create and manage a treatment schedule and build patient relationships.
