About The Position

FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in Rahway, New Jersey, and surrounding communities. Care is delivered in patients’ homes and senior living settings, including independent living, assisted living, and memory care communities, using FOX’s Geriatric House Calls™ model. Clinicians help patients improve mobility, independence, wellness, longevity, and quality of life through evaluations, individualized plans of care, and evidence-based treatment. The role offers autonomy to create and manage a treatment schedule and build lasting patient relationships. Clinicians may also provide telehealth and other virtual care when appropriate. FOX describes its practice as clinician-operated, interdisciplinary, and committed to meaningful relationships, evidence-based care, and abolishing ageism. Full-time, part-time, and PRN/FOX Flex employment pathways are available; the posting gives a standard New Jersey full-time base pay range of $70,000–$102,000 annually, with part-time and PRN compensation prorated. Requirements

Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
Degree from an accredited Physical Therapy program.
Basic computer and EMR proficiency.
Current CPR certification.
Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
Ability to drive to patient homes and senior living communities.
Ability to wear personal protective equipment when required.

Responsibilities

Provide one-on-one physical therapy to older adults in private residences and senior living communities.
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
Provide therapy that supports patient wellness, longevity, mobility, and functional independence.
Document care in FOX’s EMR system in accordance with regulatory standards.
Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and fitness specialists.
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
Provide telehealth or virtual care when appropriate.
Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
