About The Position

Adoration Home Health is seeking a dedicated Home Health Physical Therapist to provide patient-centered care in the Morgantown, West Virginia coverage area. The role offers one-to-one patient care, flexible scheduling options, independent work with team support, and opportunities for advancement. The position is full-time and includes a $20,000 sign-on bonus for external candidates only.

The Physical Therapist evaluates and treats patients based on physician orders, establishes measurable goals, and develops plans to support patients’ independence and function. The role also involves coordinating with the treatment team, physicians, patients, and families; documenting services; and supervising Physical Therapist Assistants and Physical Therapy Aides in accordance with the state practice act.

Requirements

• Graduate of a Physical Therapist education program approved by CAPTE.
• Current Physical Therapist license in the state of practice.
• American Heart Association CPR certification; certification may be offered and completed upon hire.
• Two years of experience in adult physical therapy.
• Excellent observation, verbal and written communication, problem-solving, and basic math skills.
• Physical therapy skills meeting the competency checklist.

Nice To Haves

• Home health experience is preferred.

Responsibilities

• Perform physical therapy evaluations and provide treatment based on physician orders, including recommending and procuring adaptive equipment.
• Establish measurable goals and develop treatment plans to help patients reach their maximum level of independence and/or function.
• Maintain communication among the treatment team, physician, patient, and family members, as applicable.
• Complete thorough and timely documentation of services provided to ensure proper billing.
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with the state practice act.
