About The Position

FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in private residences and senior living communities in Oakhurst, Ocean, Eatontown, and surrounding New Jersey communities. Using FOX’s Geriatric House Calls™ model, clinicians help patients improve mobility, independence, wellness, and quality of life. Clinicians may also incorporate telehealth and other virtual care into their schedules. The role offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and practice in a community-based, interdisciplinary setting. FOX offers full-time, part-time, and PRN/Fox Flex arrangements. Full-time roles are salaried with benefits; part-time roles are salaried with benefits eligibility based on a minimum-hours requirement; PRN/Fox Flex roles are paid per unit. Requirements

Valid Physical Therapy license in the state or states of practice, or eligibility to obtain one.
Degree from an accredited Physical Therapy program.
Basic computer and EMR proficiency.
Current CPR certification.
Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
Ability to drive to patient homes and senior living communities.
Ability to wear PPE when required.

Responsibilities

Provide one-on-one physical therapy to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
Document care in FOX’s EMR system in accordance with regulatory standards.
Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
Provide telehealth or virtual care when appropriate.
Create, manage, and adjust a treatment schedule.
Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
