Physical Therapist – Walcott, IA
About The Position
FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in Walcott, Iowa, and surrounding communities. Care is delivered in private residences and senior living settings, including independent living, assisted living, and memory care, using FOX’s Geriatric House Calls™ model. The role focuses on helping patients improve mobility, independence, wellness, and quality of life through evaluations, individualized plans of care, and evidence-based treatment. Physical Therapists can create and manage their own treatment schedules and build lasting relationships with patients. Telehealth and other virtual care may be incorporated when appropriate. Clinicians work in a collaborative, interdisciplinary setting and document care in FOX’s EMR system. FOX offers full-time, part-time, and PRN/Fox Flex employment pathways; compensation and benefits depend on employment type. H1B sponsorship is available for qualified clinicians, and new graduates may be eligible for the Emerging Professionals Mentor Program.
Requirements
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
- Ability to wear PPE when required.

Responsibilities
- Provide one-on-one physical therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Provide therapy that supports patient wellness, longevity, mobility, and functional independence.
- Document effectively in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities.
