About The Position

FOX Rehabilitation is hiring licensed Physical Therapists to provide one-on-one care to older adults in private residences and senior living communities in Muscatine, Iowa, and surrounding communities. Using FOX’s Geriatric House Calls™ model, clinicians help patients improve mobility, independence, wellness, and quality of life. Care may take place in private residences, independent living, assisted living, or memory care settings, and may include telehealth or other virtual care when appropriate. The role offers clinicians autonomy to create, manage, and adjust their treatment schedules, build lasting patient relationships, and collaborate with an interdisciplinary team. FOX offers full-time, part-time, and PRN / FOX Flex employment pathways. Eligible roles include benefits, and new graduates may be eligible for FOX’s Emerging Professionals Mentor Program. Requirements

Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
Degree from an accredited Physical Therapy program.
Basic computer and EMR proficiency.
Current CPR certification.
Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
Ability to wear PPE when required.

Responsibilities

Provide one-on-one physical therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
Document effectively in FOX’s EMR system in accordance with regulatory standards.
Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
Provide telehealth or virtual care when appropriate.
Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
