About The Position

FOX Rehabilitation is hiring licensed Physical Therapists to provide one-on-one care to older adults in Hollywood, Florida, and surrounding communities. Care is delivered in private residences and senior living settings, including independent living, assisted living, and memory care, through FOX’s Geriatric House Calls™ model. Clinicians help patients improve mobility, independence, wellness, and quality of life. The role offers autonomy to create and manage a treatment schedule and build lasting patient relationships. Telehealth and other virtual care may also be incorporated when appropriate. FOX is a clinician-operated practice focused on meaningful relationships, evidence-based care, and abolishing ageism. Physical Therapists work in a collaborative, interdisciplinary environment with PTs, OTs, SLPs, and Fitness Specialists. Employment pathways include full-time, part-time, and PRN / FOX Flex arrangements; benefits and compensation vary by eligibility and employment type. Qualified clinicians may be eligible for H1B sponsorship, and new graduates may participate in FOX’s Emerging Professionals Mentor Program. The position requires meeting physical demands associated with in-home and community-based therapy, including lifting, CPR, patient transfers and mobility assistance, repositioning patients, extended standing and walking, stair climbing, driving to care locations, and wearing PPE when required. New employees must be cleared through the Florida Background Check Clearinghouse.

Requirements

Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
Degree from an accredited Physical Therapy program.
Basic computer and EMR proficiency.
Current CPR certification.
Ability to meet the physical demands of in-home and community-based care, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly; performing CPR; providing moderate to maximum assistance with patient transfers and bed mobility; repositioning patients; standing and walking for extended periods; climbing stairs; performing therapeutic activities; driving to patient homes and senior living communities; and wearing PPE when required.
New employees must be cleared through the Florida Background Check Clearinghouse.

Responsibilities

Provide one-on-one physical therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
Provide therapy sessions that support wellness, longevity, mobility, and functional independence.
Document effectively in FOX’s EMR system in accordance with regulatory standards.
Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
Provide telehealth or virtual care when appropriate.
Create and manage a treatment schedule and build patient relationships.
