About The Position

PruittHealth is hiring a licensed Physical Therapist to help patients restore mobility, improve function, and achieve greater independence. The role is based at a skilled nursing facility in Louisville and involves providing rehabilitation and therapy services to patients with orthopedic, neurological, and mobility-related conditions. Compensation is up to $130,000 annually, based on experience, schedule, and location. The position is scheduled PRN. Requirements

Graduate of an accredited Physical Therapy program.
Current Physical Therapist (PT) license in the state of practice.
Strong clinical, communication, and documentation skills.
Foreign-trained applicants must possess credentials deemed substantially equivalent by the applicable credentialing organization.

Nice To Haves

Experience in skilled nursing, rehabilitation, home health, or post-acute care is preferred.
New graduates are welcome where applicable.

Responsibilities

Evaluate patients with orthopedic, neurological, and mobility-related conditions.
Develop and implement individualized treatment plans.
Improve patients’ strength, balance, gait, endurance, and overall functional mobility.
Monitor patient progress and modify interventions as needed.
Complete timely and accurate clinical documentation.
Collaborate with physicians, nurses, therapists, and caregivers.
Educate patients and families on rehabilitation goals and home exercise programs.
