Per Diem Physical Therapist - Flexible Hours! (Bridgeport, CT)
About The Position
Constellation Home Care is seeking a per diem Physical Therapist for its home health program. This field-based role partners with patients as they work to restore movement and function, with the goal of maximizing each patient’s quality of life and movement potential. The organization emphasizes clinical excellence, compassionate and respectful care, and service to patients in the community setting. The Physical Therapist develops and implements patient-specific care based on assessments and prescribed plans, educates patients and caregivers, and maintains timely, compliant clinical documentation. The position serves patients in their homes and requires reliable transportation and travel to patient locations.
Requirements
- Licensed Physical Therapist in the state of employment.
- At least 1–2 years of clinical experience; new Physical Therapy graduates will also be considered.
- Reliable transportation and willingness to commute to provide in-home patient care.
- Basic knowledge of the iOS operating system.
- Willingness to complete the organization’s in-house and field orientation and training.
Nice To Haves
- Home care experience preferred.
- OASIS experience strongly desired.
Responsibilities
- Initiate, review, and implement each patient’s physical therapy plan of care based on the initial assessment and prescribed plan of care.
- Educate patients and caregivers about the disease process, medications, plan of care, and individualized treatment plan.
- Maintain and process documentation in compliance with agency standards using the electronic medical record system.
- Submit clinical documentation, time records, and other information within the required time frame.
- Travel to patients’ homes to provide care.
