About The Position

FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in Culpeper, Virginia, and surrounding communities. Care is delivered in patients’ homes and in Senior Living Communities, including independent living, assisted living, and memory care settings, with telehealth or other virtual care available when appropriate. Using FOX’s Geriatric House Calls™ model, clinicians help patients improve mobility, functional independence, wellness, longevity, and quality of life.

The role offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and make clinician-driven decisions in a collaborative, interdisciplinary practice. FOX offers full-time, part-time, and PRN/Fox Flex employment pathways. New graduates are eligible for the Emerging Professionals Mentor Program. Compensation varies by experience and employment type, and benefits are available for eligible roles.

Requirements

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
• Ability to wear personal protective equipment (PPE) when required.

Responsibilities

• Provide one-on-one physical therapy to older adults in private residences and Senior Living Communities, including independent living, assisted living, and memory care settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
• Document effectively in FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
• Provide telehealth or virtual care when appropriate.
• Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
