About The Position
FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in Bay Ridge, NY, and surrounding communities. Care is delivered in private residences and senior living settings, including independent living, assisted living, and memory care, using FOX’s Geriatric House Calls™ model. Clinicians help patients improve mobility, independence, wellness, longevity, and quality of life through evaluations, individualized plans of care, and evidence-based treatment. Telehealth and other virtual care may also be incorporated when appropriate. The role offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and collaborate with an interdisciplinary team. FOX provides full-time, part-time, and PRN/Fox Flex employment pathways; compensation and benefits depend on employment type and eligibility. The position includes opportunities to earn additional compensation through referrals, clinical instruction, mentoring, patient supervision, care above quota, and content creation. New graduates may be eligible for FOX’s Emerging Professionals Mentor Program, and H1B sponsorship is available for qualified clinicians. Clinicians must meet the physical demands of in-home and community-based care, travel to patient homes and senior living communities, and wear PPE when required.
Requirements
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly; performing CPR; and providing moderate to maximum assistance with patient transfers and bed mobility.
- Ability to drive to patient homes and senior living communities.

Responsibilities
- Provide one-on-one physical therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Provide therapy that supports patient wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Travel to patient homes and senior living communities as part of the Geriatric House Calls™ model.
- Meet physical demands including patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, performing therapeutic activities, and CPR.
- Wear PPE when required.
