Physical Therapist - Eastchester, NY
About The Position
FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in Eastchester, NY and surrounding communities. Care is delivered in private residences and Senior Living Communities, including Independent Living, Assisted Living, and Memory Care, using FOX’s Geriatric House Calls™ model. Clinicians help patients improve mobility, independence, wellness, and quality of life. The role offers autonomy to create and manage a treatment schedule and build lasting patient relationships. Telehealth and other virtual care may also be incorporated when appropriate. FOX is a clinician-operated practice focused on meaningful relationships, evidence-based care, and abolishing ageism. Clinicians work in a collaborative, interdisciplinary environment and have opportunities for professional growth and clinician-driven decision-making. The posting describes full-time, part-time, and PRN/FOX Flex employment pathways. Full-time roles are salaried with benefits; part-time benefits eligibility depends on meeting a minimum-hours requirement; FOX Flex compensation is per unit. H1B sponsorship is available for qualified clinicians, and new graduates may be eligible for the Emerging Professionals Mentor Program. The New York standard base pay range for a full-time role is $75,000–$115,000 annually, with an opportunity to earn more. Part-time and FOX Flex compensation is prorated. Eligible-role benefits include medical, dental, and vision insurance, a 401(k) with company match, unlimited continuing education access, and professional certification reimbursement. Additional earning opportunities may include referral bonuses and incentives, clinical instruction, mentoring, supervising assistants, care above the bi-weekly quota, and creating written or social media content.
Requirements
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, CPR, patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
- Ability to drive to patient homes and senior living communities.
- Wear PPE when required.

Responsibilities
- Provide one-on-one physical therapy to older adults in private residences and senior living settings.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Deliver therapy sessions supporting wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Create and manage a treatment schedule and build patient relationships.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
