About The Position

FOX Rehabilitation is hiring licensed Physical Therapist Assistants to provide hands-on care to older adults in Oceanside, NY, and surrounding communities. Using FOX’s Geriatric House Calls™ model, PTAs deliver care in patients’ homes and senior living communities while working closely with a supervising Physical Therapist and an interdisciplinary team that includes PTs, OTs, SLPs, and Fitness Specialists. The role offers meaningful one-on-one patient interaction, mentorship, and opportunities to develop clinical skills, caseload, and career. FOX offers full-time, part-time, and PRN/FOX Flex employment pathways; benefits depend on role eligibility and, for part-time roles, minimum-hours requirements.

Requirements

Valid Physical Therapist Assistant license in the state(s) of practice, or eligibility to obtain one.
Graduate of an accredited Physical Therapist Assistant program.
Current CPR certification.
Basic computer skills and proficiency in Microsoft Office.
Ability to meet the physical demands of in-home and community-based care, including lifting, patient transfers and bed mobility, performing CPR, and extensive walking.
Ability to drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
Ability to wear PPE when required.

Responsibilities

Deliver individualized treatment plans established by the supervising Physical Therapist, adjusting activities based on patient response and progress.
Instruct, motivate, and safeguard patients through therapeutic exercises and functional activities, observing and tracking their progress.
Document visits accurately and on time in FOX’s EMR system, in accordance with FOX’s Clinical Documentation standards.
Stay connected with patients, POAs, and community referral sources, and help coordinate coverage when needed.
Provide in-home and community-based care, including visits to senior living communities.
